Based on observation, interview, record review, the licensee did not comply with the section cited above in TWO of TWO LIC 9227 forms are missing which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee does not have a LIC 9227 for two out of two children onsite. Licensee will update children’s files and place the LIC 9227 in the file by POC date of 12/17/2025.
Based on observation, interview, record review, the licensee did not comply with the section cited above in TWO out of TWO which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee does not have a 15-minute check log for two out of two children onsite. Licensee will create a 15-minute check sleep log and place in the file by POC date of 12/17/2025.
Based on observation and record review, the licensee did not comply with the section cited above where both staff inside the facility were not able to show a current status for CPR training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of current CPR training, and submit to LPA by POC date.
