PEREZ FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in ensuring the outdoor play area is free of any items that would hinder with the health & safety of the children in care. There were rusted metal plates in teh backyard inside the play house and one next to the play house which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that the metal plates will be removed and pictures of the cleaned up back yard will be submitted to the department by the POC due date.

Based on observation and interview, the licensee did not comply with the section cited above in ensuring that the license is posted in a prominent place and was available upon request which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that a written and signed statement will be submitted to the department stating that the License will be posted in a prominent place visible to parents and anyone entering the facility. Licensee agrees to email the statement to LPA by the POC due date.

Based on record review, the licensee did not comply with the section cited above inhaving a signed LIC 9227 for C1 on file for the department to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that a signed LIC 9227 will be submitted to the department by the POC due date.

Based on observation the licensee did not comply with the section cited above in having a functioning carbon monoxide device present at the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a new device and send a picture of the receipt to the department by teh POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in renewing teh CPR & First AId which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to renew the CPR & First Aid training by teh POC due date and submit the certificate to the department by the POC due date.

Based on record review, the licensee did not comply with the section cited above in having her certification for Mandated Reporter completed and available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the trainings for Mandated Reporter and mail a copy to the department by the POC due date.
