Based on observation, the licensee did not comply with the section cited above in ensuring the stone pillar and three retaining wall planter locations are made inaccessible foot holds which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will make the required corrections to make the pool inaccessible when unattended and submit pictures of the correction to the department by the POC due date.

Based on interview and record review, the licensee did not comply with the section cited above in obtaining TB test for S1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get S1 complete their TB test and submit the results to the Department by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on interview and record review, the licensee did not comply with the section cited above in creating a sleep log for C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a Sleep log for C1 and document the checkt and submit a completed log to the department by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on interview and record review, the licensee did not comply with the section cited above in having proof of MMR, TDAP, and Flu vaccines for S1 available for the department to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit MMR, TDAP, and Flu vaccine for S1 to the Department by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in having Mandated Reporter training certification completed for herself which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Training (General And Child Care) and submit proof to the department by the due date.
