Based on LPAs observation, the licensee did not comply with the section cited above where a kitchen cabinet's lock was broken where cleaning agents were observed to be present, and an outside shed doors were left without a lock where dangerous chemicals were stored which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee placed a lock securing the shed doors outside. Licensee states that the broken lock in the kitchen will be replaced by POC date.
Based on record review the licensee did not comply with the section cited above where the licensee did not maintain C1, and C2's immunizations. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will provide an updated immunization record for C1, and C2 by POC date.
Based on observation during inspection], the licensee did not comply with the section cited above the backyard cabinet had bug spray cans which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Backyard cabinet had Cutter Backyard Bug Control spray and OFF bug deterrent spray easily accessible to children in care. LPA removed the items and handed to assistant to be placed in an area inaccessible to children in care. Items were removed at time of inspection and assitant was informed of the reason as to being removed.
Based on obseration, the licensee did not comply with the section cited above in pool fence has a chain wrapped around the fence with a lock on the gate. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: licenee will send LPA Flores photo of correction via email by 4/18/23
Based on record review, the licensee did not comply with the section cited above in licnesee's mandated reporter is expired as of 3/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: licensee will Complete her Mandated reporter course and send LPA Flores via email a photo of completed certificate by 4/18/23
