Based on interview and record review, the licensee did not comply with the section cited above in that Licensee has an expired mandated reporter child care provider training on file and no general training on file. Assistant has not completed mandated reporter training. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of training to the Department for mandated reporter training for licensee and assistant by due date 11/28/2022.

Based on records review, the licensee did not comply with the section cited above in that a CPR and First aid was completed but was not completed by and EMSA approved trainer, American Red Cross or American Heart association training provieder, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof to the Department of CPR and First aid training completed by an approved trainer, by due date 11/28/2022.

Based on interview and record review, the licensee did not comply with the section cited above in that assistant did not have record of immunizations against measles on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof of immunization for measles for assistant by due date, 11/28/2022.
