Based on record review, the licensee did not comply with the section cited above where C1 was missing their LIC9227. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will develop a LIC9227 for C1, and provide to LPA by POC due date. Additionally, Licensee states that they will conduct in-service training and provide proof of such by POC date.
Based on record review the licensee did not comply with the section cited above where C1 does not have documented 15 minute checks which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will document C1's sleep in 15 minute intervals, and develop a log and provide to LPA by POC date. Additionally, in-service training will be conducted with all staff so that proper documentation can be made. The in-service training is due by POC date.
Based on observation the licensee did not comply with the section cited above where C2, and C3 were observed to be asleep in high chairs. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will conduct in-service training with all staff on the cited regulation, and provide to LPA by POC date.
Based on observation the licensee did not comply with the section cited above where C4 was observed to be placed in a high chair after having a disagreement with another child. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will conduct in-service training on the cited regulation with all staff and provide proof of such to LPA by POC date.
