Based on nterview and record review, the licensee did not comply with the section cited above in keeping a sleep log for the infant enrolled at the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the sleep log for C1 and submit the completed log for a week to the department by the POC due date. Sample Sleep log was provided.

Based on record review, the licensee did not comply with the section cited above in obtaining a TB test for A1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a TB test for A1 and submit the result to the department by the POC due date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in obtaining clearance for the Adult residing at the facility which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of teh receipt for LIVE SCAN (LIC 9163) form to the department by the POC due date and have LIC 9224 signed by enrolled families and kept in each child's file. Upon receipt of a Type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the LIC 9224 Acknowledgement of Receipt of Licensing Reports was given to Licensee Jo Ann Evans.

Based on interview and record review, the licensee did not comply with the section cited above in not conducting her fire drill every 6 months, the last emergency and fire drill was conducted on 05/08/2022 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct an emergency and fire drill by teh POC due date and submit to the department.

Based on observation, the licensee did not comply with the section cited above in keeping cleaning supplies present in the child care restroom locked and out of reach of children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to place all items that need to be out of reach of children locked. Licensee will submit a picture of the correction to LPA by the POC due date.

Based on record review, the licensee did not comply with the section cited above in not having immunization records for the 3 children out of 3 children present at the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the immunization record and submit a copy to the department by the POC due date.

Based on record review, the licensee did not comply with the section cited above in having PM 286 completed for all 3 out of 3 children present at the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete PM 286 and submit a copy to the department by the POC due date.
