Based on record review, LPA found that the last drill conducted and documented was on 12/6/23. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will conduct and document a disaster drill and provide proof of such to LPA by POC date.
Based on observation the licensee did not comply with the section cited above where an Infant, Child #2 (C2) was observed sitting in a rocking bouncer/bassinet. This posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will discontinue to use the rocking bouncer, and provide a statement of understanding regarding the cited regulation to LPA by POC date.
Based on observation the licensee did not comply with the section cited above when LPA observed Child One (C1) asleep in a bassinet. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will not allow children to sleep in bouncers, bassinets, etc. Licensee will provide a statement of understanding of the cited regulation to LPA via email by POC date.
Based on observation the licensee did not comply with the section cited above where LPA found ammunition stored together with other locked firearms in a locked safe. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will obtain a safe and store the ammunition separately from the firearms in the facility and provide proof of such to LPA by POC date.
