Based on interview and record review, the licensee did not comply with the section cited above in ensuring staff #1 was cleared and associated to the facility prior to working with the children which poses an immediate health, safety or personal rights risk to persons in care. Civil Penalties have been assessed.

Resolution: Licensee will submit LIC 9188 requesting a transfer for staff #1.

Based on observation, interview, and record review, the licensee did not comply with the section cited above inensuring staff #1's documentation of clearance was present on file for the department to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a statement stating that Licensee will ensure that staff clearance documents are present on file prior to them working at the childcare.

Based on record review, the licensee did not comply with the section cited above in having LIC 627 for C1 on file for the department to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit the completed LIC 627 for C1 to the department by the POC due date.

Based on observation, interview, record review, the licensee did not comply with the section cited above in conducting a fire drill every 6 months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire drill and submit the proof by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in S1 completing her Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit the certificate for Mandated Reporter training for S1 by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in Licensee and S1 completing their Preventive Health & Safety Training and her EMSA approved CPR & First Aid training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get S1 complete Preventive Health and Practices training and EMSA approved CPR & First Aid training for Licensee and S1 and submit the proof to the department by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in S1 having proof of her MMR & TDAP vaccines which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of MMR & TDAP vaccines for S1 and submit it to the department by the POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in 3 out of 4 children's files reviewed was missing LIC 995A which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain signed and completed LIC 995A for C1, C3, and C4 and submit to teh department by teh POC due date. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b), that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.
