Based on observation, interviews, the licensee did not comply with the section cited above in not having a pool alarm which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee she will purchase a pool alarm and provide LPA Perry with receipt and photo as well as start a daily documentation log for inspeciation daily of the pool.
Based on observation, interview and record review, the licensee did not comply with the section cited above as 2A-10BC fire extinguisher observed was last purchased on 4/23/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase new 2A-10BC fire extinguisher and send LPA P. Ochoa proof of purchase by POC date.
Based on observation, interview and record review, the licensee did not comply with the section cited above as the licensee, assistant and licensee's husband do not have proof of Mandated Reporter training completion, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and staff will comeplete Mandated Reporter training and send LPA P. Ochoa completed certificate POC date.
Based on observation, interview and record review, the licensee did not comply with the section cited above as assistant does not have LIC 9052 and LIC 9108 in assistant file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send the complted LIC 9052 and LIC 9108 to LPA P. Ochoa by POC date.
Based on observation, interview and record review, the licensee did not comply with the section cited above as licensee did not have a roster present for LPA to obersver during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete child roster and send it to LPA P. Ochoa by POC date.
Based on observation and record review, the licensee did not comply with the section cited above in that Licensee is not documenting infants sleep and did not have log available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will start documenting children's nap 4/202/2023 and have logs available for review.

The one on file was taken online- not EMSA aprroved and expired on 6/16/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will contact MAOF to register for CPR class- once registered they will provide proof of registration to LPA,
Based on observation, the licensee did not comply with the section cited above in that Licensee did not have the required A 210BC fire extinguisher. The extinguisher on site is smaller and has not been serviced which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will buy the required A210BC fire extinguisher and provide proof of purchase to LPA by POC due date of 4/28/2023
