Based on observation, interview and record review, the licensee did not comply with the section cited above as all staff did not have proof of mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA proof of completed Mandated reporter Child Care Provider (AB1207) training for all staff by the plan of correction date.

Based on interview and record review, the licensee did not comply with the section cited above as all staff did not have proof of immunization record on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA proof of their Measles immunzation record and assistant's Measles, TDAP, TB and influenza immunizations by the plan of correction date and keep a copy in staff files.

Based on observation, the licensee did not comply with the section cited above as LPA observed one fire extinguisher to have a service date from 2018 and a second fire extinguisher that the Licensee stated was purchased recently to not have a purchase receipt or service tag which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will either obtain the purchase receipt for the existing fire extinguisher or purchase a new fire extinguisher and submit proof to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above in that the Licensee did not have proof of current mandated reporter training available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will either locate the proof of current Mandated Reporter training and submit to LPA by POC due date or will take the Mandated Reproter training and submit the proof of completion to LPA by POC due date.
