Based on observations], the licensee did not comply with the section cited above, LPAs Kruz Long and Veronica Martinez-Garza observed Child #3 to be covered in a blanket while sleeping in the crib. which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: During inspection, Licensee removed blanket from crib. Correction completed.
Based on observation, the licensee did not comply with the section cited above. There were 4 infants, 1 toddler and 1 Staff (Licensee) present during inspection, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, Declaration (LIC855) will be completed with a plan to ensure Staffing ratio and Capacity is in compliance with Title 22 Regulations. Once completed, Licensee will provide a copy to the department by the POC date.

Resolution: Per Licensee, an Infant Sleep Chart for all Infants will be completed and a copy will be provided to the department by the POC date.
