Based on observation and record review, the licensee did not comply with the section cited above in LPA observed that the co-licensee is missing proof of the MMR vaccine which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of the MMR vaccine to LPA by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed the licensee and co-licensee's mandated reporter training expired on 01/18/25 and 01/19/25 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of the mandated reporter training to LPA by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed child 1 (C1) is missing LIC 627 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of a complete LIC 627 to LPA by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed the licensee last serived the fire extinguisher on 09/16/24, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of service for the fire extinguisher to LPA by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed that the licensee last documented the fire and disaster drill on 09/20/24 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of the fire and disaster drill to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed child 1 (C1 thru 4) did not have LIC 627 on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of completed LIC 627 to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in licensee Maria Poot-Herrera does not have proof of MMR and Tdap vaccines which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of MMR and Tdap will be submitted to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed child 3 (C3) and C4 did not have LIC 995A which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of completed LIC 995A to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in Licensee last purchased the fire extinguisher on 12/20/22 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will either service the fire extinguisher or purchase a new one and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in licensee does not have proof of the disaster drill which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of a disaster drill to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in both licensee's pediatric CPR/FA expired on 02/24/23 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of the pediatric CPR/FA will be submitted to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in 1 out of 1 missing carbon monoxide detectors which poses/posed a potential health, safety or personal rights risk to persons in care. No carbon monoxide detector was observed in the home by LPA.

Resolution: Licensee Evelyn Walldez will obtain and install a carbon monoxide detector.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 Mandated Reporter Certificates for both licensees, which poses/posed a potential health, safety or personal rights risk to persons in care. Both licensees did not have proof of Mandated Reporter Training.

Resolution: Both licensees, Evelyn Walldez and Maria Poot-Herrera will complete Mandated Reporter Training and print certificates for their files.

Based on observation, the licensee did not comply with the section cited above in 2 out of 2 fire extinguishers and smoke detectors which poses/posed a potential health, safety or personal rights risk to persons in care. Fire extinguisher was last serviced on 3/24/20 as shown on service tag, and althoujgh there was a smoke detector in the home, there was no test button to test the device to see if operable.

Resolution: Licensee will obtain and install a new smoke detector and service the fire extnguisher.
