ORNELAS FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in licensee's grandson Jeramiah Ornelas lives in the home and is not fingeprint cleared which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Jeramiah Ornelas will get fingerprinted on 09/20/2024 and will submit proof to LPA by POC date. LPA provided LIC 9163.

Based on observation, the licensee did not comply with the section cited above in licensee's fire extinguisher does not have a service tag or proof of purchase which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will service the fire extinguisher and will submit proof to LPA by POC due date.

Based on records review of last service on fire extinguisher tag dated 06/16/2021. This poses a potential health and safety risk to children in care.

Resolution: Per Licensee she will take the fire extinguisher to get service on or before POC due date and will send LPA Garcia via email a copy of the service tag by 04/12/2023.

Based on records review of child #1's file no record of sleeping logs; Time of each 15 minute check, Date were available for review. This poses a potential health and safety risk to children in care.

Resolution: Per Licensee she will began to visually check sleeping infants every 15 minutes and will record visual check on a sleep log and will submit copies of logs to LPA Garcia via email by POC due date 04/12/2023.
