Resolution: Per Licensee, both her and the assistant will take the Pediatric CPR/FA through an approved vendor (EMSA, American Red Cross, or American Heart Association) and will submit proof to LPA by POC due date.

Resolution: Per Licensee, she will submit proof of Influenza vaccine or declination letter for assistant and TB test clearance for adults #2 and #3 to LPA by POC due date.
