HERRERA FAMILY CHILD CARE

Based on LPA Garcia observing that the Licensee was not present upon LPAs arrival, and Assistant was caring and supervising nine (9) chidren in care. This poses and immediate health and safety risk to children in care.

Resolution: During LPAs visit Assistant contacted Licensee via telephone to advise LPA was present at 9:24 am, Licensee arrived. Per Licensee, she will comply with Tittle 22 regulations a self-declaration was obtain from Licensee which documents compliance plan.

Based on records review and Licensee disclosure, infant in care sleeping log is not documentated. This poses a potential health and safety risk to children in care.

Resolution: Per Licensee, she will begin to document infant sleeping log every 15 minutes upon physically checking on infant and will submit records of sleeping log to LPA Garcia via email by POC due date 06/30/22

Based on records review of Licensee and Assistant no certificate of course completion is on file for Child Abuse Mandated Reporter Training AB1207. This poses a potential health and safety risk to children in care.

Resolution: Per Licensee, she and her Assistant will go online and complete the Child Care Provider training AB1207 and a copy of the certificates will be submitted via email to LPA Garcia by POC due date of 06/30/22.

Based on records review of Licensee and Assistant file no immunization records are on file. This poses a potential health and safety risk to children in care.

Resolution: Per Licensee, she and her Assistant will obtain immunization records and a copy of the records will be submitted to LPA Garcia via email by POC due date of 06/30/22.
