Based on observation, the licensee did not comply with the section cited above by failing to maintain a clean and organized facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email photos of the facility that is clean and organized.

Based on observation, the licensee did not comply with the section cited above in that: 1) the outdoor play area has animal droppings and other items that may cause hazard to children, 2) lower portion of the bathroom walls for children needs to be fix/repair, 3) trash bin in the bathroom for children shall have a lid cover which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email photos of the corrections on the outdoor play area, bathroom walls for children, and a trash bin with a cover.

Based on record review, the licensee did not comply with the section cited above by failing to maintain immunization records for herself at the facility, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit copies of immunizations to the Department and retain copies at her facility no later than 07/01/2022.

Based on record review, the licensee did not comply with the section cited above by failing to maintain updated/renewed CPR/First Aid certification at the faciilty, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to obtain copies of the renewed CPR/First Aid certification card showing new expiration date no later than 07/01/2022.

Based on record review, the licensee did not comply with the section cited above failing to maintain a copy of the Identification and Emergency Identification (LIC700) in one child's file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit a copy of the completed LIC700 to the Department and maintain a copy in the child's file no later than 07/01/2022.
