During the inspection of the backyard, LPA observed a plastic play structure that has a broken step on the bottom that is wide enough for a child's foot to get caught. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will remove, fix, and or replace the play structure. Licensee will send pictures to LPA for proof by the due date.
During review of records, it was discovered the the Licensee did not have any required licensing forms on file for any of 6 children that were present during the inspection. Licensee stated that she provided the parents with new forms to complete as a lot of the families had moved, however per the licensee, the children have been enrolled from about 2 months to 2 years. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will have all required licensing forms completed by all the parents of the children currently enrolled, plus the immunization records, and send proof of all documents obtained for each child to LPA. Licensee can take pictures of all forms and send them to LPA as well. All documents to be submitted to LPA by due date 05/07/2024.
