In attendance at this meeting was: Regional Manager (RM), Susie Fanning, Licensing Program Manager (LPM), Kari McWilliams, Licensing Program Analyst (LPA), Xona Xayavong, Director, Cassandra Torrez, Site Supervisor, Danielle Van Zwaluwenburg, and Board Member, Cynthia Zellman. The purpose of this meeting was to discuss the recent violations of Title 22 Regulations. Director Cassandra Torrez has implemented a plan of correction since being cited. The following deficiencies were cited on July 23, 2024, by the Department during an unannounced complaint inspection: · Type A – CCR 101229(a)(1) – Children left the playground through an unsecured gate in the play yard. Children were found by another staff. Another incident occurred where a child was left in the play yard while their class was inside. · Type B – CCR 101212(d)(1)(C) – The Facility did not report incidents to Parent/Guardians of children. Incidents were also not reported to the Fresno Regional Office. Director agrees to the following: · Director is hereby reminded that she is required to ensure that the health, safety, and personal rights of children in care are protected at all times. · Facility will stay in compliance with California Code of Regulations Title 22 Division 12 Chapter 1 Regulations, as well as California Health & Safety Code laws related to child care center, at all times. Licensee was informed of childcare training videos available on the Community Care Licensing website at www.ccld.ca.gov.
