GOMEZ, CLARIBEL FAMILY CHILD CARE

Based on interview with Licensee, she was not documeting the infant safe sleep checks every 15 minutes for labored breathing and signs of distress. Licensee has three infants (INF1, INF2, INF3) in care, including one infant under 12 mos. at the time of the inspection. Licensee did not have the LIC9227 form completed for INF3. This poses a potential risk to the health, safety or personal rights of children in care.

Resolution: Licensee states she will document and provide LPA a copy of safe sleep log and forms, as applicaple to the CCL Fresno Child Care Office by 06/26/2025 and will continue to document each infant's sleep log checking for labored breathing and signs of distress.

Based on record review and interview with Licensee, none of the children in care have documentation of the emergency information form, consent for emergency medical treatment, notification of parent's rights, parent notification of additional children in care, affidavit regarding liability insurance and immunizations. This poses a potential risk to the health, safety, or personal rights of children in care.

Resolution: Licensee states she will provide documentation of documentation of the emergency information form, consent for emergency medical treatment, notification of parent's rights, parent notification of additional children in care, affidavit regarding liability insurance and immunizations for all children to the CCL Fresno Child Care Office 06/26/2025 and will continue to document each child’s immunizations for all children enrolled.

Based on record review and interview with Licensee, Licensee does not have documentation the Children's Roster. This poses a potential risk to the health, safety, or personal rights of children in care.

Resolution: Licensee states she will provide a copy of the Children's Roster to the CCL Fresno Child Care Office by 06/26/2025 and will maintain a record of children she has in her care.

Based on interview and observation, licensee did not have an assistant present and there were 9 children in care (3 infants, 3 preschool age and 3 school age). Without an assistant, the maximum number of children for whom care may be provided is six children when three infants are in care. This poses an potential risk to the health, safety, or personal rights of children in care.

Resolution: Licensee had a infant picked up while LPA was present and was back in compliance with ratio and capacity. Licensee states she will submit a written statement with a plan to adhere to the regulation regarding capacity and ratio.

Based on interview and observation, licensee did not have an assistant present and there were 9 children in care. This poses an potential risk to the health, safety, or personal rights of children in care.

Resolution: Licensee had a child picked up while LPA was present and was back in compliance with ratio and capacity. Licensee stated that going forward she will ensure that she complies with ratio and capacity requirements of a small family child care home if/when no assistant provider is present or if she leaves an assistant to care for children while she is not present. Deficiency cleared during today’s inspection.
