Based on record review, the licensee did not comply with the section cited above. LPA observed that Staff #1 Mandated Reporter Training expired 1/2024, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will have assistant take Mandated Reporter Training and submit proof to CCLD by POC date of 2/14/2025.

During inspection, the cupboard doors located under the sink were not locked. When LPA opened the doors to inspect the area, LPA observed there to be Clorox bleach. The cupboard doors did have a child lock on one of the handles, however it was not locked, therefore accessible to the children. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed product and stored it outside in the garage where the children do not have access to. Licensee stated she will review Child Care Licensing regulation CCR 101238 and submit a statement of understanding which includes how the facility will prevent similar incidents in the future. Statement of understanding will be sent to LPA via email by end of workday 02/16/2024.

During record review and observation, the fire drill log that Licensee provided appeared to be altered. It appears that Licensee wrote a 4 over the 3 in 2023 to indicate 2024 for fire drill, smoke alarm drill, and carbon monoxide drill. The same thing appears for the year 2022, Licensee wrote a 3 over the 2 in 2022 to indicate 2023. There is also no documented time of day that the fire drill was conducted. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill by POC due date 02/16/2024 and write down the time the drill was conducted and how many children were present. Licensee will write a statement of understanding the importance of the fire drills and how she will make sure fire drills are conducted and documented accurately. Licensee will submit document to LPA via email by POC due date 02/16/2024 via email.

Based on interviews and record reviews, Licensee did not have immunization records for five out of the fifteen children that are enrolled in the facility. The Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will do a file review and get all the children's missing immunization forms. Licensee will write a plan of correction stating how this will not happen again and submit proof to LPA via email by POC due date 2/29/2024.

Resolution: Licensee will read safe sleep regulations and submit a written statement on what was learned and how she plans on correcting the deficiency. Licensee will submit POC by 02/16/2024.
