Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above in LPA observed in accessible kitchen and master bedroom multiple cabinets containing 5 kitchen knives, 3 Mylanta bottles, loose screws and nails, screw drivers, flat heads, OTC and prescription medications, Ajax, travel size hand sanitizers, Got2b hair dye and hardware tools which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will install safety latches for cabinets in kitchen and master bedroom and provide pictures to LPA via email by end of business 04/25/2025.

Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above in LPA observed in accessible backyard a wooden patio gate attached to the North side of the home. The wooden patio gate entrance lock is loose and is not secured. The attached gate against the wall of the home is loose, wobbling and is bending inwards which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fix wooden patio gate and lock and send pictures and videos to LPA by end of business 04/25/2025.

Based on LPA records review and Licensee interview, the licensee did not comply with the section cited above in Licensee's assistant/daughter does not have Mandated Reporter Training, immunization records (MMR and Tdap), Tb test, LIC9052 and LIC9108 on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated assistant/daughter will complete the requested documents and send copies to LPA via email by end of business 04/25/2025.

Based on observation, the licensee did not comply with the section cited above LPA observed a faulty kitchen cabinet safety lock not working properly which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to fix or replace the safety lock to the kitchen cabinet under the sink or remove all detergents to a higher location out of the reach of children. Meanwhile Licensee has blocked the area.

Based on observation and record review, the licensee did not comply with the section cited above Licensee CPR/First Aid training is expired as of 9/2022, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit evidence of EMSA CPR/First Aid training course renewal by 03/07/23.

the licensee did not comply with the section cited above LPA obsersved infant sleeping with the door closed in a play pin with a bottle and blanket which which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to read safe sleep regulations and applies them to her care with infants. LPA supplied a copy with Licensee and discussed it during Staff interview portion of the inspections. Door was kept open while infant slept and Items in the crib were removed from the sleeping infant.

Based on observation, the licensee did not comply with the section cited above Licensee stated she had not been documenting sleep for infant in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee agreed to start a new infant sleep log today. LPA observed Licenesee check and document sleeping infant.
