Based on observation and interview, the licensee did not comply with the section cited above. LPA arrived at 12:40 PM and entered the home at approximately 12:45 PM. LPA observed licensee caring for nine (9) children with no assistant present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed licensee was back in ratio at 12:37 PM when assistant arrrived.

Based on observation, interview and record review, the licensee did not comply with the section cited. Licensee was unable to provide documentation that shows fire/disaster drills are being conducted at least once every six months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit proof that shows she is conducting drills at least once every six months by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide documentation that shows fire/disaster drills are being documented at least once every six months. Licensee provided LPA documents that shows the last fire/disaster drill was documented and conducted on 10/15/2019 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to document drill and provide proof to LPA by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee did not have PUB 394 Notification of Parent Rights posted which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: At 1:58 PM LPA observed assistant posting the document next to the facility license on the wall inside the day care room.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During file reviews, At 1:42 PM LPA observed Child (C2) did not have form LIC 627 Consent for Emergency Medical Treatment which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have form complete and placed inside child (C2) file and provide proof to LPA by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews at 1:37 PM, LPA observed child (C1), (C5), (C7) did not have immunization records inside their file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to obtain copies of the childrens immunization and place the form inside the childrens file by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee provided LPA a roster but the roster that was provided only had 3 out of the 9 children present during todays inspection listed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete a current roster for children that are enrolled and provide LPA proof by POC due date.
