Based on observation, interview, and record review, the licensee did not comply with the section cited above. While touring the home at 1:15 PM LPA observed staff #1 caring for three children inside the play room. Based on record review, staff #1 was not associated to the facility and licensee did not obtain a transer request for staff #1 which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed staff #1 leave the facility. Licensee agreed not to have staff #1 present until staff #1 has been cleared and associated to the facility.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee and two assistants did not have a current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to attend the training. Liicensee agreed to have staff #1 and staff #2 attend the training and send proof of all certifications to LPA by POC due date
