Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed staff are not documenting infant safe sleep once every 15 minutes during nap times. Staff stated they write the information on the white board and at the end of day they transfer the information into the ProCare app. Staff were unable to provide proof of 15 minute nap logs which poses/posed a potential health, safety or personal rights risk to persons in care.

While touring the facility at 9:40 AM, LPA observed eight children sitting at the tables eating without any staff present. LPA walked around the corner and found the staff in charge of the classroom with the eight children eating assisting a child inside the restroom which poses an immediate health, safety or personal rights risk to persons in care.
