Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs (1) No child(ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation. This requirement is not met evidenced by: Based on staff interviews and staff stating once C1 entered the hallway, C1 was not visible and was unsure if C1 went into the kitchen or not, this is an immediate risk to the health and safety of children in care.

Activities (c) A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center. This requirement was not met by: on 5/5/2023, at 1:56 PM, LPA observed S2 alone with 26 children napping on their cots inside Pre K room #6 classroom. This poses an immediate health and safety risk to children in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During record review, Director stated the last disaster drill was conducted on May 12, 2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above. While touring inside of the facility at 8:57 a.m. LPA observed 10 preschool age children present with 1 staff. LPA observed the bathroom door open and inside, LPA observed Peroxide Cleaner that reads "Keep out of reach of children" that was accessible to children in care. Also, while touring the the classrooms #1 and #2, LPA observed a can of Raid Ant Killer inside an unlocked cabinet under the children's water faucet that reads "Keep out of reach of children." Inside classroom #6 there were 15 children with two staff. LPA observed bleach water inside and unlocked cabinet on the lower shelf that children had access to. bleach water bottle reads "corrosive, skin and eye irritant, inhalation irritant, and harmful if ingested" which poses an immediate health, safety or personal rights risk to persons in care.

During record review, Director was unable to provide proof showing disaster drills are being documented which poses/posed a potential health, safety or personal rights risk to persons in care.

While touring the outside patio area at 9:45 a.m. LPA observed sharp rotted wood and rotted nails around the window and doors. Assistant director stated preschool children utilize the patio area which poses an immediate health, safety or personal rights risk to persons in care.

This requirement was not met by: At 12:32 PM on 5/5/2023, LPA observed S4 exit the facility, leaving S2 and S3 with 28 children inside Pre- k room #6. Children were observed walking, running, sitting on chairs, and some children were on their cots. This poses an immediate health and safety risk to children in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. 2 out of 4 teachers present at the time of inspection did not have proof of immunization of measles, pertussis, and influenza which poses/posed a potential health, safety or personal rights risk to persons in care.
