Licensee was unable to provide LPA with their immunization records during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit immunization records for child #4 (C4) to LPA by POC due date via email at dean.thompson@dss.ca.gov, mail. or text.
