Based on observation, the licensee did not comply with the section cited above. Upon entry into the family child care home at approximately 8:35 a.m., LPA observed the stairs leading to the second and third level of the home was not fenced or barricaded allowing children to have access to the entire home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed S1 barricading the stairs during the inspection.
Based on observation, the licensee did not comply with the section cited above. While touring the home, LPA observed an unlocked cabinet underneath the kitchen sink. Inside the unlocked cabinet LPA observed Clorox Bleach Foamer, Ammonia, Cleaner & Degreaser, and CLR Grease Magnet that reads "Harmful if Swallowed" and "Keep Out of Reach of Children." LPA also observed an unlocked drawer inside the kitchen with multiple steak knives that were approximately 6 to 8 inches long. Near the stairs, LPA observed a pill bottle that children in care had access to with approximately 20-30 Pravastatin pills inside. There was also multiple liquor bottles in the bar area which is located on the second level of the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed S1 barricading the stairs during the inspection and removing children from the second level of the home.
Based on observation, the licensee did not comply with the section cited above. While touring the outside area of the home, LPA observed the pool in the back yard was not locked and two sections were not attached which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Submit proof to LPA by POC due date showing the pool gate is completely closed so that children do not have access.
Based on observation, the licensee did not comply with the section cited above. LPA observed three children playing inside the garage upon arrival which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed assistants removing the children from the garage during the inspection.
Based on interview, the licensee did not comply with the section cited above. Assistants present could not provide documentation showing a fire/disaster drill is being conducted at least once every six months which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Provide documentation to LPA showing proof drills are being condcuted and documented at least once every six months.
Based on observation and interview, the licensee did not comply with the section cited above. Upon arrival, assistant present stated the licensee was not home. Assistant called licensee on the phone and licensee stated she would not be home today which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above. Assistants present could not provide proof of their AB1207 Mandated Reporter certification when requested which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: At 10:40 a.m. S1 and S2 was able to find their file and provided LPA with their mandated reporter certifications.

Based on observation, interview, record review, the licensee did not comply with the section cited above. Immunization records for child #1 (C1), child #2 (C2), and child #3 (C3) were not available which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed there were no gates at the stairs leading into up to the dining room, family room, and upstairs bedrooms. In the area children had access to, LPA observed numerous bottles of liquor, Ortho Home Defense canister, and Lysol Canister which reads "keep out of reach from children" which poses an immediate health, safety or personal rights risk to persons in care. This is a repeat violation.
Resolution: Licensee corrected this deficiency after LPA indicated to her that the stairs needed to be barricaded.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Assistants present did not have a file for review. Assistants were missing immunization records, mandated reporter training certifications, and LIC 9108 statement acknowledging requirement to report child abuse form which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit files for assistant to LPA by POC due date via email or mail to the Orange County Regional office.
