Based on observation, interview, record review, the licensee did not comply with the section cited above by allowing uncleared adult, Assistant #2 (A2) to provide care and supervision to children, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: A2 was removed from facility while LPA was present. Licensee will complete training at https://ccld.childcarevideos.org/family-child-care-providers/supervising-children-in-family-child-care/ and submit statement indicating understanding of requirement that all adults providing care must be fingerprint cleared before entering facility to LPA Cha via email by 8/14/2025.
Based on interview and record review, the licensee did not comply with the section cited above by allowing Assistant #1 (A1) to provide care and supervision to children without completing the Mandated Reporter training course, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: A1 enrolled in Mandated Reporter course during inspection. A1 will send proof of completion of Mandated Reporter training to LPA Cha via email by 8/20/2025.
