LPA met with the Director of Early Childhood Education Michelle Truong and explained the nature of the visit. Present on this visit were 14 Staff and 111 Preschool Children. The facility operates from Monday to Friday 8:00 AM to 6:00 PM. On Monday, October 2, 2023 at 3pm in classroom 111, S1 and S2 led their students to the outdoor playground. C1 and C2, were left behind in the classroom for less than two minutes. S1 and S2 realized that the children were not in the line and returned immediately to the classroom to get the children. S1 reported the incident to the Head of School, Deepali Deshmukh. Deepali informed C1 and C2 authorized representatives. Director of Early Childhood Education, Michelle Truong, called in the Oakland South Child Care Regional Office on 10/3/2023 at 3pm to report the incident. On Thursday, October 5, 2023 at 10:07 AM, LPA Estoesta receive the completed LIC 624 via email. On 10/4/2023, the Director conducted a Staff Meeting to go over supervision, ratios, name to face, and counting the students at various points of transitions. The Director stated to continue to spot check classrooms to ensure the ratios are maintained and all the safety protocols are followed, and also continue to review all safety protocols during regular staff meetings. On 10/18/2023, the Director will conduct a Staff meeting and will include safety protocols. LPA conducted staff interview, a health and safety inspection, record review and obtained copies of the Plan of Correction (POC) documentation. LPA Estoesta informed the Director and that this report dated 10/18/2023 included a Type A Citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. An Immediate Civil Penalty was also assessed. Also, LPA Estoesta informed the Director to provide a copy of this licensing report dated 10/18/2023 that document of any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Director of Early Childhood Education Michelle Truong.
