Based on record review, at 10:15 am, the Licensee's Assistant Provider - Yunqing Ye has a current Criminal Background Clearance with Per ID of 4600656748, however, she is not associated in the Guardian Roster. As per the licensee, Yunqing Ye started vowrki ng in the home since March 2025. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a copy of the LIC 9182 CRIMINAL BACKGROUND CLEARANCE TRANSFER REQUEST and advised the Licensee to complete the form and mail the copy to the Oakland Regional Office address on or before the POC due date. LPA recommended to the Licensee to review the video Background Check Requirements for Caregivers https://ccld.childcarevideos.org/family-child-care-providers/background-check-requirements-for-caregivers/.

Based on record review, at 10:30 am, C1 did not have a completed LIC 9227 on C1's file. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a copy of LIC 9227 to the Licensee to complete and submit a copy to Oakland Regional Office on or by POC due date.
