Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee CPR, 1st aid certificate has been expired.

Resolution: The licensee needs to submit class registration by due date.

Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Helper fingerprint is not associated to the facility

Resolution: The facility submitted LIC9182 today. The deficiency has been corrected.

Based on record review, the licensee and helper did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and helper need to submit current AB1207 certificate by due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by having a person without fingerprint clearance in daycare, which poses an immediate health, safety, or personal rights risk to persons in care.

Resolution: The licensee will ensure that the person is not in the childcare home until fingerprint clearance is approved. The licensee will email LIC 9163 to the LPA by 2/3/25.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by having children in an off-limit area, also no one was supervising them, which poses an immediate health, safety or personal rights risk to persons in care

Resolution: The licensee will make sure the off-limit areas are properly barricaded / locked and no children in there. She will ensure the children are actively supervised all the time.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by having chemicals and the item which poses/poses a potential health, safety, or personal rights risk to persons in care.

Resolution: During the LPA visit the licensee removed the chemical and the knife and placed them on the high shelf.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by leaving children (one hour) with a person who does not have CPR which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will pause her class in the library until a new assistant (who has all required records including CPR) stars on 2/10/25 and provides an update to the LPA via e-mail on 2/7/2025.
