Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above. LPA observed in accessible fenced backyard a trampoline. LPA observed the condition of the trampoline in normal wear and tear condition that is unsafe for children use which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's stated they are going to replace the wear and tear fabrics and send pictures to LPA for confirmation. Licensee’s understand the trampoline is not for use until LPA confirmation.
Based on LPA childrens records review, the licensee did not comply with the section cited above in child 01 and child 02 were missing LIC613A and LIC9149 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit copies of the requested forms to LPA via email by end of business on 02/07/2025.
