Based on LPAs observation, the licensee did not comply with the section cited above in there were items accessible to children in the hallway bathroom, such as shaving cream, razors, lysol and air freshener, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will remove the items from the bathroom and keep in another area of the home that is not accessible to children in care. Licensee will also watch CCLD video on Locks and Inaccessibility Requirements and will submit a written statement of what she learned to CCLD by the POC due date of 10/29/21.

Based on LPAs record review, the licensee did not comply with the section cited above in that licensee does not document fire drills, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will document fire drills going forward. Licensee to submit written documentstion of last fire drill to CCLD. Licensee will also watch CCLD video on Disaster Planning and FIre Safety and will submit a written statement of what she learned to CCLD by the POC due date of 10/29/21.

Based on LPAs record review, the licensee did not comply with the section cited above in that Licensee was unaware she was to check on infant while sleeping and document infant's sleep, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will review information on safe sleep provided to her and will submit a written statement that she understands the safe sleep requirements to CCLD by the POC due date of 10/29/21.

Based on LPAs record review, the licensee did not comply with the section cited above in that Licensee has not completed the Mandated Reporter Training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she may have taken required training at Access. If she can not produce a certificate of the completed training, she will take the training online and submit proof of completion by the POC due date of 10/29/21.

Based on LPAs record review, the licensee did not comply with the section cited above in that Licensee has not renewed her CPR/First Aid Certification, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she has training scheduled for next month to renew her certification. Licensee will submit proof to CCLD by the POC due date of 11/19/21 of completed training.
