Based on record review, Licensee could not provide proof of the 15 minute check logs for all children under 24 months. Licensee stated she has not been completing the 15 minute checks for any children. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will start completing 15 minute sleep logs for all infants in care under 24 months. Licensee will email LPA proof of sleep log completion by the due date. Licensee understands that she must complete 15 minute sleep logs for all children under 24 months moving foward.
Based on record review, the Licensee was unable to provide LPA with a file for her assistant and stated assistant has not completed mandated reporter training. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have her assistant complete the mandated reporter training and create a file for her with all required licensing forms. Licensee will email proof of all documents to LPA by the due date.
