Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above in LPA observed a play structure consisting of 3 swings attached to a climbing structure and a slide in accessible backyard. The climbing structure is tilted and is not sturdy and moves when tugged against which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated the play structure will be removed off the premises. Licensee will submit pictures to LPA via email by end of business on 03/28/2025.
