Resolution: Licensee stated Licensee will have C4's parent/authorized representative complete LIC627 and submit a copy to LPA via email by 12/26/2025.

Based on observation. LPAs observed two infants in swings in one of the bedrooms. Licensee stated she was putting both infants down for a nap. LPAs discussed safe sleep regulations with Licensee. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will review the safe sleep material LPAs provided her during today's inspection and write a statement about what she learned and how she will follow safe sleep regulations going forward.

Resolution: Licensee stated she will work with each child's authorized representative to obtain the required forms and provide copies of each child's file to Community Care Licensing by 1/3/23.
