Based on LPA records review and interviews with the Licensee and assistant, the licensee did not comply with the section cited above in child 04 is missing the following: immunization records, LIC282, LIC627, LIC995, LIC700 and LIC9150 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will provided copies of the completed requested forms for child 04 and submit to LPA via email by end of business on 03/14/2025.
Based on review of facility records, LPA observed Licensee and Staff #1 did not have current AB 1207 Mandated Reporter certification. This poses a potential health, safety or personal rights risk to children in care.
Resolution: Licensee stated that she and Staff #1 will renew the required AB 1207 Mandated Reporter training as required and provide documentation of current certification to Community Care Licensing by 3/16/22.
