, LPA was informed through interviews that staff did not ensure the classroom was free from choking hazards, due to child choking on the strap due to strangulation. This posses a potential risk to the health, safety and/or personal rights of children in care.

Based on LPAs observations and interviews, the license did not comply with the section cited above. On 07/01/2025, LPA observed children napping in cots blocking exit doors which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs interviews, observations, records review and records obtained, the license did not comply with the section cited above. Licensee did NOT ensure the outdoor play area was free of hazard resulting in child 07 sustaining an injury which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs interviews and records obtained, the licensee did not comply with the section cited above. On 06/23/2025, preschool classrooms were served undercooked hamburger patties which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations, records review and interviews. The facility failed to post their Disaster Plan in a prominent area were staff would be able to access it as it was stored at an off-site location. Furethermore, interviews disclosed that the facility did not conduct fire drills and did not instruct staff in their duties and responsibilities under the plan, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA’s interviews and records obtained, the licensee did not comply with the section cited above staff accepts children with signs of illness into care in which poses/posed a potential health, safety or personal rights risk to persons in care.
