Based on observation, the licensee did not comply with the section cited above several toys were observed to have spiderwebs and broken pieces exposing sharp edges. During the visit Licensee ensured to dispose of the broken non operating toys and clear all spiderwebs which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was advised to dispose of broken toys and clear all the spider webs during the licensing visit. License has disposed and cleared the spiderwebs from children’s toys while LPA's were present.
Based on record review, the licensee did not comply with the section cited above S2 did not have a current up to date mandated reporter certification which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated S2 will conducted the mandated reporter training and provide proof to LPA by COB 06/20/2025.
