On 02/10/23, Director contacted LPA, to advise the facility would need to close for one day to address a mice issue at the facility, which poses/posed a potential health, safety or personal rights risk to children in care.

The facility submitted an Unusual Incident Report dated October 19, 2022, which reported that the classroom door was open and Child #1 (C1) exited the room without staffs awareness and was found by parent on the playground and returned to room, which poses an immediate health, safety or personal rights risk to persons in care.
