Based on record review, the licensee did not comply with the section cited above 15 miute sleep checks are being conducted until children are 12 months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee wll begin conducting 15 minute sleep checks until infants in care are 24 months. Proof willbe provided to LPA via text messge by COB 06/17/2025

Based on record review, the licensee did not comply with the section cited above C1 and C3 did not have LIC282 affidavit regarding liability insurance which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will obtain a completed and signed LIC282 Affidavit regarding liability insurance and maintin a copy in the childs file. Proof will be provided to LPA by COB 07/17/2025.

Based on observation, the licensee did not comply with the section cited above the carbon monoxide tester is not in working condition which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA witness licensee replace the batteries putting the carbon monoxide detector is working order.
