MENDOZA FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above LPA observed three exposed circular concrete blocks securing the swing set which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will cover the three concrete circular blocks with either woodchips or black cushion that could absorb a fall, making the blocks inaccessible and harmless to children in care. Proof will be provided to LPA.

Based on observation,interview, and record review, the licensee did not comply with the section cited above S2 did not have a file readily available for viewing which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, S2 will create and maintain a file making it readily avaialble for viewing for CCL. Proof will be provided to the LPA.

Based on record review, the licensee did not comply with the section cited above C5 did not have a signed and dated LIC627 Consent for emergency medical treatment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will have C5's legal guardian sign and dated the consent for emergency medical treatment. Proof of signature will be provided to LPA by 01/28/2025.

Based on interview and record review, the licensee did not comply with the section cited above C1-C4 did not have current up to date sleep logs. Licensee stated, they physically check however there was no documentation of the sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will begin douing infant safe sleep logs ever 15 minutes and document. Proof will be provided to the LPA on 02/21/2025.
