Based on record review, the licensee did not comply with the section cited above C1 and C3 did not have any sleep logs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will conduct 15 sleep check and document properly. Licensee was reminded tha sleep logs are required to be conducted and documented every 15 minutes until the child the child reaches 24 months. Proof of completed document will be submitted to LPA by COB 10/24/2025
Based on (record review, the licensee did not comply with the section cited above C3 did not have a LIC9227 Sleep plan on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee ststed, they will ensure an LIC9227 is documented and filed for C3. LPA advised Licensee that infant sleep plans LIC9227 are required for infants until the child reaches 12 months. Proof of completed document will be submitted to LPA by COB 10/24/2025
