Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 6 files missing vaccines which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will obtain proof of vaccines and email LPA.

Resolution: Licensee stated S2 was only supporting for the day but will have S2 get fingerprinted and will submit proof of completion to LPA via email.

Based on record review, the licensee did not comply with the section cited above S2 did not have an LIC9052 notice of employees rights readily available for viewing which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain a signed and dated LIC9052 for S2 that must remain in S2's file. Proof will be provided to LPA.

Based on record review, the licensee did not comply with the section cited above, licensee has not conducted a fire drill since October 2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will conduct a fire drill and provide documentation to LPA via email. Proff shall be submited by 08/22/2024.

Based on record review, the licensee did not comply with the section cited above S2 did not have a current up to date mandated reporter certification poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, S2 will complete mandated reporter training. S2 will provide licensee proff of completed certification and proff will be provided to LPA via email.

Based on record review, the licensee did not comply with the section cited above S2 did not have required immunizations readily availabe for viewing which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will obtain a copy of S2 immunizations. Proof will be provided to LPA via email.

Based on record review, the licensee did not comply with the section cited above, licensee did not have a file for S2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create and maintain all required documentation for S2. Proof will be provided to LPA by 09/06/2024.

Based on record review, the licensee did not comply with the section cited above C3 did not have immunizations in their file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: License stated they will obtain record of C3's immunizations and place immunization records in C3s file. Proof will be provided to LPA via email by 09/06/2024

Based on record review, the licensee did not comply with the section cited above C1 and C2 did not have a completed LIC700 ID. Emergency Information form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain a completed and signed LIC700 I.D. Emergency Information for C1 and C2. Proof will be provided to LPA via email by 08/22/2024.

Based on record review, the licensee did not comply with the section cited above C1 and C2 did not have 15 minute sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will begin doing sleep logs. Licensee understands they must observe and document sleeping infants every 15 minutes. Proof will be provided to LPA ny 09/06/2024

Based on record review, the licensee did not comply with the section cited above in which child #7 does not have form LIC 995A (Parent's Rights) on file available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Aubree Hunt agrees to have form LIC 995A (Parent's Rights) completed for Child #7 and submit a copy of the documented checks to Community Care Licensing by 09/29/22. Ms. Hunt was reminded that there would be a $100.00 Civil Penalty for every day the plan of correction is late, per day.

Based on records review licensee did not comply with the section cited above in whch which poses Child #7 does not have form LIC 9227 (Individual Infant Sleeping Plan which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Aubree Hunt agrees to obatin a copty of form LIC 9227 (Individual Infant Sleeping Plan) for Child #7 and submit a copy to Community Care Licensing by 09/29/22. Ms. Hunt was reminded that there would be a $100.00 Civil Penalty for every day the plan of correction is late, per day.

Based on record review, the licensee did not comply with the section cited above in which child #7 does not have a 15 check log on file available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Aubree Hunt agrees to conduct a 15 minute safe sleep infant check for infants (Child #7) and submit a copy of the documented checks to Community Care Licensing by 09/29/22. Ms. Hunt was reminded that there would be a $100.00 Civil Penalty for every day the plan of correction is late, per day.

Based on record review, the licensee did not comply with the section cited above in which there is no facility roster available for review during this inspection.which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Aubree Hunt agrees to complete and submit a facility roster to Community Care Licensing by 09/29/22. Ms. Hunt was reminded that there would be a $100.00 Civil Penalty for every day the plan of correction is late, per day.

Based on record review, the licensee did not comply with the section cited above in whch Child #3, Child #6 & Child #7 do not have their immunization records available for review durint this inspections which poses a potential health, safety or personal rights risk to persons in care.
