Resolution: Licensee, Aubree Hunt agrees to have form LIC 995A (Parent's Rights) completed for Child #7 and submit a copy of the documented checks to Community Care Licensing by 09/29/22. Ms. Hunt was reminded that there would be a $100.00 Civil Penalty for every day the plan of correction is late, per day.
Based on records review licensee did not comply with the section cited above in whch which poses Child #7 does not have form LIC 9227 (Individual Infant Sleeping Plan which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee, Aubree Hunt agrees to obatin a copty of form LIC 9227 (Individual Infant Sleeping Plan) for Child #7 and submit a copy to Community Care Licensing by 09/29/22. Ms. Hunt was reminded that there would be a $100.00 Civil Penalty for every day the plan of correction is late, per day.
Based on record review, the licensee did not comply with the section cited above in which child #7 does not have a 15 check log on file available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee, Aubree Hunt agrees to conduct a 15 minute safe sleep infant check for infants (Child #7) and submit a copy of the documented checks to Community Care Licensing by 09/29/22.
