Based on observation, the licensee did not comply with the section cited above, there were cleaning compound underneath the bathroom sink that should have had a mechanism to prevent access by children which posed a potential health, safety or personal rights risk to persons in care. Licensee also had several uncovered electrical plugs which poses a potential health and safety or personal rights isk to persons in care.

Resolution: Licensee placed the safety door knob on the bathroom door to prevent access by children in care.

Based on record review, the licensee did not comply with the section cited above C1,and C3 did not have a completed consent for emergency meical treatment LIC627 on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain a completed copy for both children and maintain in the childrens files. Proof will be provided to LPA by COB 05/16/2025.

Based on record review, the licensee did not comply with the section cited above C1, did not have their immunization records on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will obtain C1 and C3's immunization records and keep record of them in their respective files. Proof will be provided by COB 05/16/2025

Based on record review, the licensee did not comply with the section cited above during file review the licensee did not have a current up to date first aid/CPR certification which poses a, potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will take the irst aid/CPR course and obtain a current certification. Proof will be provided to LPA by COB 05/16/2025.

Based on observation lit candles that were accessible to children in care. Licensee removed the candles like LPA's were present. LPA's also observed several plugs without safety coverings. Licensee covered all accessible plugs making them inaccessible while LPA's were present. , the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the candles and covered all accessible plugs while LPA's were present.

Based on observation During the facility tour LPA's tested the carbon monoxide tester that did not appear to be in working order, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will replace the batteries in the carbon monoxide detector and proof of working order will be provided to LPA by 02/22/2024.

Based on record review C2 did not have any documentation of sleep logs available, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin doing sleep logs and proof will be provided to LPA via email by 02/22/2024.

Based on record review, C2 did not have immunizations on file. the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a copy of C2's immunization record and proof will be provided to LPA via email by 02/22/2024.

Based on record review, C3 did not have an LIC627 Consent to emergency medical treat authorization on file.,the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated the correct forms will be given to the parents dated and signed and proof will be provided to LPA via email by 02/22/2024.

Based on observation, the licensee did not comply with the section cited above, there were eight children present total with no school age children present making the licensee out of ratio. Licensee called an assiatnt to come help and assist. Assistant arrived while LPA's were present. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: While LPA's were present an assistant arrived to help the licensee with the children in care, making them in ratio.

Based on record review, the licensee did not comply with the section cited above in Child 1-10 did not have the a signed Notification of Parent Rights, LIC995A which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit signed copies of Notification of Parent Rights, LIC995A for Child 1-10 as proof of correction.

Based on interview and record review, the licensee did not comply with the section cited above in Child 1-12 did not have proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit copies of immunizations on PM286 for Child 1-12 as proof of correction.

Based on observation, interview and record review, the licensee did not comply with the section cited above in the Licensee did not have a current roster, LIC9040 with the children who were present listed on Roster that was available which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update and include Child 1-10 and all of the children who are currently enrolled on Roster, LIC9040 and submit a copy as proof of correction.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Child 1-10 did not have Affidavit, LIC282 on file for Child 1-10 and no proof of liability coverage which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtained Affidavit, LIC282 for Child 1-10 and submit copies to the department as proof of correction.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Staff #1 did not have approval of criminal record transfer. Present during todays visit, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to not have Staff #1 not present until verification of transfer of criminal record clearances has been approved, and submit a written statement of understanding to the department.

Based on observation, the licensee did not comply with the section cited above in downstairs bedroom was a knife, adult scissor and other hazardous items, in the bathroom by the downstairs bedroom was cleaning supplies, in the backyard there was several items ladder, glass, metal stakes, lug wrench and other items, artificial grass/carpet that can cause tripping hazardous all accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove all hazardous items or make inaccessible submit pictures to the department as proof of correction.

Based on record review, the licensee did not comply with the section cited above in that Child 1-10 did not have Identification and Emergency Information, LIC700 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain completed Identification and Emergency Information, LIC700 for Child 1-10 as proof of correction.
