Based on record review the licensee did not comply with the section cited above in that one infant was missing the safe sleep plan (LIC9227) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide the LIC9227 form to authorized caregiver to fill out and will place it in the child's file. Licensee will send a copy to the department by the POC due date.

Based on observation, the licensee did not comply with the section cited above in that one infant was found sleeping in a swing placed in a pack and play with aproximately 15 loose toys and two blankets which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will no longer have loose items in the crib with sleeping infant. Licensee will review provided safe sleep regulations and will write a statement of understanding of the regulation and will submit to the department by POC due date.

Resolution: Licensee, Catarina Coronado agrees to submit copies of current mandated reporter certificates for herself and her assistant by 03/31/23. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b, that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.
