Based on record review the licensee did not comply with the section cited above in that one infant was missing the safe sleep plan (LIC9227) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide the LIC9227 form to authorized caregiver to fill out and will place it in the child's file. Licensee will send a copy to the department by the POC due date.
Based on observation, the licensee did not comply with the section cited above in that one infant was found sleeping in a swing placed in a pack and play with aproximately 15 loose toys and two blankets which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will no longer have loose items in the crib with sleeping infant. Licensee will review provided safe sleep regulations and will write a statement of understanding of the regulation and will submit to the department by POC due date.
Based on observation, the licensee did not comply with the section cited above in that one sleeping infant was found sleeping in a pack and play with blankets drapped over each of the four sides of the pack and play which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will no longer have blankets draped on the sides of the pack and play. Licensee will review provided safe sleep regulations and will write a statement of understanding of the regulation and will submit to the department by POC due date.
Based on observation, the licensee did not comply with the section cited above in that one sleeping was found in a pack and play with a blanket covering their face which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will no longer have blankets in the pack and play with the sleeping infant. Licensee will review provided safe sleep regulations and will write a statement of understanding of the regulation and will submit to the department by POC due date.
Based on observation, the licensee did not comply with the section cited above in that one infant was found sleeping with an attached pacifier which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will removed the pacifier attachment from the infant. Licensee will review provided safe sleep regulations and will write a statement of understanding of the regulation and will submit to the department by POC due date.
Based on record review, the licensee did not comply with the section cited above in which licensee and assistant both have expired Mandated Reporter Certificates which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee, Catarina Coronado agrees to submit copies of current mandated reporter certificates for herself and her assistant by 03/31/23. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b, that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.
