Based on observation, the licensee did not comply with the section cited above in that the Carbon Monoxide detect present was non-operable which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain a new operable Carbon Monoxide Detector and send pictures of installed locations to the Department by 6/30/2022
Based on observation, the licensee did not comply with the section cited above in that LPAs observed a baby walker outside in the day-care outdoor play area. No child was observed in the walker however the Licensee stated that she uses it from time to time, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee corrected during visit, removed the baby walker from the day-care area and placed in a off limit area, which is inaccessible at this time.
Based on ecord review, the licensee did not comply with the section cited above in that Assitant #2, did not have proof of certificate of completion and was observed providing care and supervision. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee agrees to have Assistant #2 complete and submit a copy of the Mandate Reporter Training Certificate to the Department by 6/30/2022.
Based on record review, the licensee did not comply with the section cited above in Assistant #2 did not have proof ot immunizations: Influenza, Pertussis and Measles, which poses a health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of immunization to the Department by 6/30/2022.
