Based on observation,interview, and record review, the licensee did not comply with the section cited above in 2 out of 2 files reviewed did not have Mandated Reporter Training completed which poses a potential health, safety or personal rights risk to persons in care. Licensee and assistant were unable to provide proof of completion at the time of the inspection.
Resolution: Licensee and assistant will complete the mandated reporter training on or before 10/03/2025 and will submit a copy of the completion certificate to the LPA as verification.
Based on observation and interview, the licensee did not comply with the section cited above. Licensee's adult grandson visits the facility periodically due to working in the surrounding area. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will get adult grandson fingerprinted and associated to the facility or she will not allow any visitors during daycare hours.
