Based on record review, the licensee did not comply with the section cited above in two out of two infants in care are missing 15 min sleep checks on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to send three days worth of sleep checks for two infants in care by 10/6/25. Send copy of sleep checks to LPA at tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above in Licensee was caring for 8 children and 3 of them were infants which puts Licensee out of ratio which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to review the regulations for ratio and write a statement and plan of action to maintain ratios for children in care. Send plan of correction to LPA by 11/4/24 to tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above in no carbon monoxide detector on site which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a carbon monoxide detector and send picture of installed device to LPA by 11/4/24. Send picture to tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above in 3 counts of not having pack and plays for infants currently in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have all parents with currently enrolled children sign the LIC 9224 within next buisness day or next day child is enrolled, Licensee will also have any parent's who enroll their children to the facility within the next 12 months sign the LIC 9224. Licensee will provide pack and plays for all infants in care. Licensee will watch safe sleep video provided and write a statement of what they learned and send to LPA Sims Via email sydney.sims@dss.ca.gov by 11/2/23

Based on observation, the licensee did not comply with the section cited above in 1 count of supervising 11 children without an assistant present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have all parents with currently enrolled children sign the LIC 9224 within next buisness day or next day child is enrolled, Licensee will also have any parent's who enroll their children to the facility within the next 12 months sign the LIC 9224. Licensee immediatley called assistant to come to facility to help supervise children. Licensee will sign agreement to not operate out of ratio and submit statement to LPA Sims via email sydney.sims@dss.ca.gov by 11/2/23

Based on record review, the licensee did not comply with the section cited above in 2 counts of licensee and assitant not having their mandated reporter training up to date which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Assistant to take mandated reporter training by 11/08/23 on mandatedreporterca.gov and email certificates to LPA Sims via email sydney.sims@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above in 3 counts of not maintaing children's immunization records in their files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain children's immunizations records by 11/8/23 and send them to LPA Sims via email sydney.sims@dss.ca.gov by 11/8/23

Based on observation, the licensee did not comply with the section cited above in 2 counts of using bassinet for infant sleeping and having a rocker sleeper which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have all parents with currently enrolled children sign the LIC 9224 within next buisness day or next day child is enrolled, Licensee will also have any parent's who enroll their children to the facility within the next 12 months sign the LIC 9224. Licensee immediatley removed rocker and bassinet. Licensee will watch safe sleep video provided and write a statement of what they learned and send to LPA Sims Via email sydney.sims@dss.ca.gov by 11/2/23
